How to submit a notice

- Your physical or electronic signature.
- Identification of the copyrighted work claimed to have been infringed, or a representative list if multiple works are involved.
- Identification of the material complained of, together with information reasonably sufficient to locate it on the site.
- Your contact details, including your name and email address.
- A statement that you have a good-faith belief that the disputed use is not authorised by the copyright owner, its agent, or the law.
- A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorised to act on behalf of the copyright owner.
Upon receipt of a substantially compliant notice, the claim can be reviewed and the material may be removed or disabled as appropriate.
Counter-notice
If material was removed because of mistake or misidentification, a counter-notice may be submitted with the information ordinarily required under the DMCA, including identification of the removed material, a statement under penalty of perjury that the removal was mistaken, your contact details, and your consent to the applicable court jurisdiction.
